WHAT IS PRP (PLATELET RICH PLASMA)?

The term PRP stands for platelet rich plasma. Platelets are one type of cell found within our blood that are like little packets of growth and healing molecules that help your body clot in response to injury and activate a localized healing response within that same area. They can be thought of as the first responders to injury and the activators of healing. The growth factors contained in the platelet act like espresso shots for cells that can repair tissue. Hence, PRP can help stimulate a damaged area that isn’t healing to help repair itself.

HOW IS PRP MADE?

- The first step in making PRP is a blood draw.
- Next, the whole blood is placed in a centrifuge which separates the cellular components from the plasma and creates cellular layers within the blood tubes. NOTE: This can be done in a lab or by an automated centrifuge. How hard the sample is centrifuged determines where the platelets end up.
- Then the platelet layer is extracted. NOTE: Removing certain fractions of the blood at this point will create different types of PRP. - Once the concentrated platelet solution is separated and extracted, it can then be injected into the area in need of treatment.

- We have a flexible lab platform, meaning we manually isolate and concentrate the PRP
- We can customize the type and dose of PRP based on the patients age, tissue type (Tendon vs cartilage vs disc) and severity of the problem
- Other clinics use a simple automated machine which cannot customize the type or concentrate of the PRP. More of a one size fits all approach. Some automated machines don’t even reach therapeutic concentration levels for PRP.

ARE THERE DIFFERENT TYPES OF PRP? 


  • There are two main types of PRP: 
    • Leukocyte-Rich PRP (LR-PRP) : Contains high concentration of white blood cells and sometimes red blood cells (see picture of standard PRP)
    • Leukocyte-Poor PRP (LP-PRP): contains low to no red or white blood cells. 
  • There are certain conditions in which one may consider using one or the other. 
    • For example, for joint or cartilage degeneration/injury, we use high dose LP-PRP (no RBCS and low WBCs) to treat joint osteoarthritis. Studies have shown that LR-PRP (High WBCs, some RBS) can cause more inflammation and may hurt synovial cells in a joint. 
    • However, when we treat in a spinal disc with PRP we now use high dose LR-PRP (higher WBCs).

Are there different concentrations? Does the PRP “Dose” matter?


  • YES, PRP DOSE IS VERY IMPORTANT FOR ACHIEVING OPTIMAL OUTCOME
  • The concentration of the platelets in PRP is a critical factor. There is low concentration, usually 1-3 times the natural concentration of platelets in your blood. Medium concentration is 3-7 times concentrated and high concentration is 7-20X. 
  • Regenexx noted dramatic differences in how well PRP can stimulate cell growth based on this concentration (Berger DR, Centeno CJ, Steinmetz NJ. Platelet lysates from aged donors promote human tenocyte proliferation and migration in a concentration-dependent manner. Bone Joint Res. 2019;8(1):32–40. Published 2019 Feb 2. DOI). 
  • For example, when it came to younger tendon cells, the concentration mattered less, but in older cells, the concentration was a key factor in getting the cells to grow and heal a defect. Hence, we often go to higher PRP concentration in middle-aged and older patients.
  • Other studies have shown that, for knee osteoarthritis, the platelet dose needs to meet or exceed 10 billion platelets delivered. This dose can only be with a proper blood draw and efficient processing.  (Bansal et al. Platelet-rich  plasma in osteoarthritis (OA0knee: Correct dose critical for long term clinic efficacy)

WARNING: Be cautious with a clinic offering PRP that only performs a low volume blood draw (10-20ml) as this WILL NOT produce therapeutic concentrations of PRP!!!
